A North Carolina CPA applying for reciprocal licensure in another jurisdiction (state) or a successful North Carolina CPA Exam candidate applying for initial licensure in another jurisdiction (state) may use this form to request that the Board provide an Exam Score Verification (numerical Exam scores only), a License Verification (license information only) or an Exam/License Verification (license information and numerical Exam scores, if on file with the Board) to the Board of Accountancy of that jurisdiction. Requesting this information does not cancel or inactivate your North Carolina CPA license. A North Carolina CPA who needs proof of licensure for an entity other than a Board of Accountancy should print their individual record from the Find CPA/CPA Firm database on the Board's website, nccpaboard.gov.
