NOTE: This form may only be used to register a firm that is legally structured as an individual practitioner (IP). This applies even if you are the firm’s only owner, shareholder, member, or partner. If your firm is organized as a professional limited liability company (PLLC), partnership (PR), limited liability partnership (LLP), or professional corporation (PA/PC), you may not use this form. You must use the registration form that corresponds to your firm’s legal structure, available on the Forms and Applications page.
